This policy applies to onforever.care, its public pages, and the patient experience survey. It does not fully describe how clinical records, billing, insurance, external telehealth platforms, or patient portals are handled.

Information the website may process

  • Basic technical information needed to deliver, secure, and maintain the website, such as IP address, device and browser type, date, time, and security logs.
  • Responses and ratings voluntarily submitted through the experience survey. The public survey is designed not to request names, email addresses, diagnoses, or treatment details.
  • Administrative access credentials, such as an authorized email address and temporary codes, only for authorized personnel.
  • Information you choose to communicate by phone, email, WhatsApp, or an external service. Those communications are also governed by the applicable provider’s policies and terms.

Do not send sensitive health information through the public survey, ordinary email, or WhatsApp. For clinical matters, use the secure channel identified by the On Forever Health Services team.

If you provide health information to request or receive care, its treatment will depend on the context, applicable law, and the clinic’s Notice of Privacy Practices.

Service providers and disclosure

We may use providers that support website hosting, security, communications, and operations. We share information only as needed to provide those services, comply with law, protect rights and safety, or respond to a valid government request. We do not sell personal health information.

We use reasonable administrative and technical measures appropriate to the nature of the information. No website, email, wireless network, or internet transmission can guarantee absolute security.

We retain information as needed for the purposes described, legal obligations, security, and dispute resolution, and then delete or de-identify it when appropriate.

  • Use only the link sent by the clinic and confirm suspicious links by calling the official number.
  • Use an updated personal device, a strong password, and, when possible, a protected private network.
  • Avoid public Wi‑Fi, shared devices, and places where others can hear or see the appointment.
  • Do not record a session without prior authorization.

You may contact the clinic to ask about your data, correct administrative information, or learn how to exercise applicable rights. Rights involving clinical records are described in the Notice of Privacy Practices.

The website is intended primarily for parents, guardians, and people seeking services. We do not knowingly ask children to submit personal information through public pages. Links to WhatsApp, maps, social media, telehealth, and other external services are governed by their own policies.
