No clinical relationship is created

Visiting the website, reading content, submitting a survey, or starting a communication does not by itself create a professional-patient relationship. Such a relationship begins only after On Forever Health Services confirms it and applicable consents and requirements are completed.

Emergencies

This website, email, WhatsApp, and scheduled appointments are not emergency services. If you or another person is in immediate danger, call 911. For emotional or mental health crisis support in the United States, call or text 988.

Appointments and telehealth

  • Availability, eligibility, appointment length, and method of care depend on the service, patient age and location, professional authorization, and clinical judgment.
  • Some evaluations or situations require in-person care, additional sessions, or rescheduling.
  • You are responsible for providing accurate information, completing requested consents, and following privacy and preparation instructions.
  • The clinic will communicate any applicable payment, cancellation, insurance, or scholarship policy separately.

Do not send sensitive health information through public or unsecured channels. By submitting a survey or other information, you confirm that you have the right to do so and that it is not unlawful, threatening, misleading, or infringing.
